This form is designed to be used by referral agencies. If you self-refer our team may need to ask you additional questions. If you are homeless, are vulnerably housed or working with a support agency please ask them to complete this form on your behalf.

Hope into Action Reading is part of the Hope into Action national network with homes in over 30 towns and cities.

On the first page of the referral form, you can select Reading (or another area) as first choice on the drop down menu under 'Which area would you like this referral to be sent to?' If the location selected as first choice has no suitable vacancies, your Referral can be forwarded to any of the other locations which you are also able to select.

Our supported accommodation

• Hope into Action has a referral process for supported accommodation.  Through our supported housing, we enable vulnerable people to live as independently as possible in the community.

• We welcome people of all faiths and none, all ethnicities and sexualities.

• We take referrals from individuals and agencies (see above).

• If you, or someone you know is seeking supported housing, then please complete an online referral

• Sadly we have more referrals than homes available but vacancies do come up so please do get in touch if you think supported accommodation is right for you or your client.

We do not offer emergency accommodation

• Our assessment process is likely to take up to a few weeks and generally includes at least 2 face to face meetings. 

• If you need help with emergency accommodation then have a look at Shelter.

• If you are concerned about someone you have seen sleeping rough in England or Wales, you can use this website to send an alert to StreetLink.
